Reservation Cancellation Form

Use this to send University Unions Hospitality an email requesting that a reservation be cancelled. Please supply as much information as possible to help us find your reservation. If you initiated or submitted your reservation through the Unions' on-line reservation system (after August 2010) you can also cancel your own reservations through that system.

Enter "NA" for any items that don't apply to you, or which you don't have.

For RSOs enter your SSB Box. For Departments - your campus mail address. For others - your postal address.

Event Information

Your event's start time and end time.

Check the room or rooms that are associated with the reservation.
